| > Clinics A note on confidentiality: All information is kept confidential, and your clinic visit falls under doctor patient confidentiality. State medical marijuana registration records are not available to doctors, only to law enforcement in the cases where they are already seeking a warrant on a specific person. You choose how much to disclose in your records. Start by reading the qualifying conditions listed on the Attending Physician's Statement and see if your condition falls within the conditions listed on the form. The Oregon Revised Statues and the Oregon Administrative Rules are laws you should familiarize yourself with before signing up. Make certain you bring in medical records. Bring in records from at least one previous doctor's visit that documents the condition which you believe qualifies you as a medical marijuana patient. In most cases, it is easiest to have your records copied or sent to you. Try calling your doctor's office and asking for the records that document the diagnosis, and that you will be in later to pick them up. If you have box-fulls of medical records, it may be simpler to visit your doctor and request a one page brief sunnary of your qualifying condition. How much you tell your primary care physician ahead of time is up to your judgement. Come to the clinic with a few pages from your medical records, current Oregon state ID, and any forms you have already filled out. | ||||||||
